High Fidelity Wraparound Intensive Care Coordination

Family & Community Support Systems (FCSS) implements the High Fidelity Wraparound (HFW) model of Intensive Care Coordination. HFW is an evidence-based, team-based collaborative process for developing and implementing individualized care plans for children with behavioral health challenges, and their families.‚Äč

The Goals of High Fidelity Wraparound are:

  • To meet the stated needs prioritized by the youth and family
  • To improve the youth and family's ability and confidence to manage their own supports and services
  • To develop or strengthen the youth and family's natural support system over time
  • To integrate the work of all child-serving systems and natural supports into one streamlined plan

The Four Phases of High Fidelity Wraparound are:

  • Engagement: Orientation of the family to the HFW process, development of the Strengths, Needs, Culture and Discovery document, identification and orientation of team members, development of crisis stabilization plan(s)
  • Planning: Development of a youth and family driven vision and plan of care that reflects the principles of HFW
  • Implementation: Activities are repeated until the youth and family's vision, and the team's mission are achieved
  • Transition: Plans are made for a transition to formal and natural supports in the community
